Group stages cause a lot of teams to shithouse the 3rd and final game. Shut up shop, or rest players and get a draw to make it through. Or some teams of lesser quality try to shut up in all 3 games and see if that will be enough.

I don't know what the answer is but knockout from the start might work, seeded of course.
